RECEIPTS REQUIRED

For eligible items already paid for. Must be signed by Nicole Vallejo. Hard copy only — no emails, no exceptions. Due the 1st or 15th, paid the 8th or 22nd.

$5–$200+

Sponsor a specific athlete's journey. Checks payable to One Step Above or OSA. Applies toward tuition unless otherwise specified on the form.
